Abstract

As will be established throughout this book, the art of transition planning for adolescents with disabilities is a complex undertaking if it is done thoroughly and appropriately. The participants in effective transition assessment, planning, and implementation span multiple disciplines and systemic structures. However, the primary focus, as across the entire service spectrum for individuals with disabilities, must rest upon serving the individual needs of the primary consumers: the youth and peripherally, the family.
