Abstract

The specific tactics used in the treatment of the bilaterally dysfunctional couple will vary with the clinical situation. In general, our overall approach is governed by the principle of sequential treatment. We have found it practical to concentrate on the treatment of one partner at a time. The dysfunction which is generally considered to have the best prognosis is treated first. Once that spouse’s symptom has been alleviated, the situation is reevaluated and the focus of treatment is shifted to the other partner’s problem. However, the active involvement of both partners in the treatment process, regardless of the focus of therapy, is a basic tenet of sex therapy. Consequently, the sequential treatment plan is discussed with the couple beforehand, since it is important to elicit the second partner’s commitment to treatment on this basis, as part of the therapeutic “contract.”
